Pediatric Hypertension Etiology of Hypertension in Children and Adolescents

While most hypertension in children has been previously considered secondary to renal, cardiovascular or endocrine etiology, a substantial number of children aged 6 to 20 years are now diagnosed with primary or essential hypertension. Hypertension in children and adolescents seems to be increasing over the past two decades. This is attributed at least in part to an increased prevalence of overweight in this population. Essential hypertension in childhood is a diagnosis arrived at by excluding the known causes of secondary hypertension. This paper will discuss the etiology of hypertension in children and adolescents. RÉSUMÉ • Alors que la plupart des causes de l’hypertension chez l’enfant ont été précédemment considérées comme secondaires à des étiologies rénales, cardiovasculaires ou endocriniennes, un nombre assez important d’enfants âgés de 6 à 20 ans sont actuellement diagnostiqués comme ayant une hypertension primaire ou essentielle. L’hypertension chez l’enfant et l’adolescent semble avoir augmenté de fréquence durant ces deux dernières décades. Cela est attribué, au moins en partie, à une augmentation de la prévalence du surpoids dans cette population. L’hypertension essentielle chez l’enfant est un diagnostic d’exclusion, après avoir éliminé les causes connues d’hypertension secondaire.
